ABSTRACT

Objectives@#. Laryngeal ultrasonography (LUS) has been suggested as an alternative diagnostic tool for unilateral vocal fold paralysis (UVFP). The present study applied LUS and quantitative laryngeal electromyography (LEMG) in female UVFP patients to investigate the pathophysiologic mechanisms of UVFP. @*Methods@#. In this cross-sectional study, vocal fold (VF) length parameters included resting and phonating VF length measured using B-mode LUS, and color Doppler vibrating length (CDVL) measured using the color Doppler mode. @*Results@#. Forty female patients with UVFP were enrolled, among whom 11 and 29 were assigned to the thyroarytenoid (TA) muscle+cricothyroid (CT) muscle group (with CT involvement) and the TA (without CT involvement) group, respectively. In the TA group, the turn frequency in thyroarytenoid-lateral cricoarytenoid (TA-LCA) on the paralyzed side, as observed through LEMG, correlated with the VF length during the resting phase (R=0.368, P=0.050) and CDVL values (R=0.627, P=0.000) on the paralyzed side. In the TA+CT group, the turn ratio in the CT muscle correlated with the normalized phonatory vocal length change (nPLC; R=0.621, P=0.041) on the paralyzed side. @*Conclusion@#. CDVL and nPLC are two parameters that can be utilized to predict the turn frequencies of TA-LCA in UVFP cases without CT involvement, and the turn ratio of CT in cases of UVFP with CT involvement, respectively. The findings suggest that LUS, as a noninvasive tool, can serve as an alternative method for assessing the severity of laryngeal nerve injury and offer valuable insights into the pathophysiology of UVFP.

ABSTRACT

Cancer is considered as one of the major diseases endangering human health in the world, it is urgent to find a safer and more efficient treatment for cancer therapy. Gene therapy with ribonucleic acid (RNA) drugs could regulate the expression of tumor related genes, and exhibit good anti-tumor therapeutic potential in preclinical and clinical trials. Based on the differences between tumor tissues and normal tissues in microenvironment signal characteristics such as pH, specific enzyme concentration or redox gradient, various microenvironment responsive nanocarriers had been studied and developed to deliver RNA drugs to tumor tissues and cells, improving the anti-tumor efficacy of RNA drugs and reducing toxic and side effects. This paper reviews the pathophysiological characteristics of tumor microenvironment and various strategies of tumor microenvironment responsive nanocarriers, in order to provide reference for the design of safe and efficient RNA drug delivery system for cancer therapy.

The need for long-term treatments of chronic diseases has motivated the widespread development of long-acting parenteral formulations (LAPFs) with the aim of improving drug pharmacokinetics and therapeutic efficacy. LAPFs have been proven to extend the half-life of therapeutics, as well as to improve patient adherence; consequently, this enhances the outcome of therapy positively. Over past decades, considerable progress has been made in designing effective LAPFs in both preclinical and clinical settings. ABSTRACT

Objective:To determine the current situation of food safety risk monitoring ability of centers for disease control and prevention (CDCs) in a province, and provide reference for strengthening the construction of food safety monitoring ability in the future. Methods:A self-administered questionnaire survey was conducted among 14 CDCs who are responsible for food safety risk monitoring in the province in 2018. Results:The ratio of food inspection staff with senior, middle, and primary professional titles was 1∶1∶1. The educational background of food inspection staff at the provincial and municipal levels was mainly bachelor degree or above, while that of county level directly under the administration of the province was mainly college degree. The age composition of the staff at the provincial level tended to be elder; however, there were more staff with less working experience at the county level. The qualification proportion of laboratory area was 50%. The distribution proportion of food inspection equipment in the CDC at all levels was 83.3%, 79.5% and 36.3%, respectively. In total, 14 CDCs completed the monitoring task as required. They were all accredited with laboratory qualifications; however, one CDC did not conduct re-evaluation after the certification of food inspection qualification expired. Each CDC ensured the accuracy and reliability of monitoring data through the supervision in food testing. Conclusion:The food safety risk monitoring ability of the CDCs in the province is continuously improved, which can meet the needs of routine monitoring. However, the composition of professional titles of food inspection staff is not very reasonable. There is still a gap between the equipment configuration and the national requirements. The ability of food inspection needs to be improved, in which quality management and laboratory qualification should be further strengthened.

ABSTRACT

With the implementation of the two-child policy and the growing demand for child health, pediatric medication has been arousing widespread social concern. To develop the drugs suitable for children, including new compounds, new specifications and new dosage forms, is urgently required for pharmaceutical researchers. In this review, several technical bottlenecks for pediatric oral liquid preparations, as well as the novel strategies involved in drug nanocrystals, self-microemulsion, ion exchange resin and Pickering emulsion were discussed, which may be benefit to play a theoretical guiding role in the research and development of children's oral liquid formulation.

ABSTRACT

Papillary thyroid carcinoma is the most common malignant tumor in the head and neck. The incidence of papillary thyroid carcinoma has been increasing in recent years. Although the prognosis of most papillary thyroid carcinoma is better, many patients have poor prognosis due to recurrence or metastasis. Molecular markers play an important role in the development, diagnosis, treatment and prognosis of papillary thyroid carcinoma. This paper reviews the progress of molecular markers related to papillary thyroid carcinoma.

ABSTRACT

Objective To compare volar plating and nonoperative treatment for senile comminuted fractures of the distal radius. Methods From October 2012 to June 2015, 46 senile comminuted fractures of the distal radius ( AO types B and C ) were treated surgically or conservatively. Twenty old patients were managed by open reduction and fixation with a volar locked plate. They were 7 men and 13 women, with an average age of 67. 9 ± 10. 7 years, including 7 cases of AO type 23-B and 13 cases of AO type 23-C. Twenty-six patients were managed with closed reduction and plaster cast. They were 8 men and 18 women, with an average age of 66. 2 ± 11. 5 years, including 10 cases of AO type 23-B and 16 cases of AO type 23-C. The 2 groups were compared in terms of follow-up time, union time and wrist functional scores and complica-tions. Results There was no significant difference between the 2 groups in follow-up time ( P > 0. 05 ) . Fractures in the 2 groups all united about 11 weeks after surgery. According to Dienst scoring for reduction, the volar plating group had 13 excellent cases, 4 good ones, 2 fair ones and one poor case, with an excellent to good rate of 85. 0% while the nonoperative group had 5 excellent cases, 10 good ones, 5 fair ones and 6 poor ones, with an excellent to good rate of 57. 7%. A significant difference was shown between the 2 groups ( P <0. 05 ) . At final follow-ups, the volar plating group had significantly lower Gartland & Werley wrist functional score ( 2. 16 ± 1. 82 ) and complication rate ( 30. 0%, 6/20 ) than the nonoperative group [ 6. 62 ± 3. 78 and 53. 8%( 14/26 ) , respectively ] ( P <0. 05 ) . Conclusion In treatment of senile comminuted fractures of the distal radius, volar locked plating may lead to better functional recovery and fewer complications than nonoper-ative management. However, one should take full account of the physical condition and preference of the patient when surgery is chosen.

ABSTRACT

Stenosis of the coronary artery has been considered as an essential component of ischemic heart disease (IHD). Consequently, revascularization [e.g., percutaneous coronary intervention (PCI) and coronary artery bypass] has been the primary therapeutic approach to IHD. Such strategy has indeed revolutionized the management of IHD patients. However, not all patients with myocardial ischemia have visible coronary stenosis. Moreover, cardiovascular events occur in nearly 20% patients with stable coronary artery disease who have undergone PCI. The recently proposed "solar system" hypothesis of IHD postulates that coronary stenosis is only one (albeit important) of its features. Mechanistic contribution and clinical implication of multiple pathophysiological processes beyond coronary stenosis are highlighted in this hypothesis. On the basis of a holistic regulation and individualized medicine, Chinese medicine (CM) has been used in the real-world setting to manage a variety of diseases, including IHD, for more than two thousands years. In this article, we summarize the evidence of CM that supports the "solar system" IHD hypothesis, and argue for a comprehensive approach to IHD. At the theoretical level, the central features of this approach include a holistic view of disease and human subjects, as well as individualized medicine. At the practical level, this approach emphasizes anoxia-tolerance and self-healing.

ABSTRACT

<p><b>OBJECTIVES</b>This systemic review evaluated the efficacy and safety of Chinese herbal medicines (CHMs) in patients with coronary heart disease (CHD) complicated with depression.</p><p><b>METHODS</b>All databases were retrieved till September 30, 2014. Randomized controlled trials (RCTs) comparing CHMs with placebo or conventional Western medicine were retrieved. Data extraction, analyses and quality assessment were performed according to the Cochrane standards. RevMan 5.3 was used to synthesize the results.</p><p><b>RESULTS</b>Thirteen RCTs enrolling 1,095 patients were included. Subgroup analysis was used to assess data. In reducing the degree of depression, CHMs showed no statistic difference in the 4th week [mean difference (MD)=-1.06; 95% confidence interval (CI)-2.38 to 0.26; n=501; I(2)=73%], but it was associated with a statistically significant difference in the 8th week (MD=-1.00; 95% CI-1.64 to-0.36; n=436; I(2)=48%). Meanwhile, the combination therapy (CHMs together with antidepressants) showed significant statistic differences both in the 4th week (MD=-1.99; 95% CI-3.80 to-0.18; n=90) and in the 8th week (MD=-5.61; 95% CI-6.26 to-4.97; n=242; I(2)=87%). In CHD-related clinical evaluation, 3 trials reported the intervention group was superior to the control group. Four trials showed adverse events in the intervention group was less than that in the control group.</p><p><b>CONCLUSIONS</b>CHMs showed potentially benefits on patients with CHD complicated with depression. Moreover, the effect of CHMs may be similar to or better than antidepressant in certain fields but with less side effects. However, because of small sample size and potential bias of most trials, this result should be interpreted with caution. More rigorous trials with larger sample size and higher quality are warranted to give high quality of evidence to support the use of CHMs for CHD complicated with depression.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To introduce practical diagnostic criterion of blood stasis syndrome (BSS), and to evaluate its reliability and validity.</p><p><b>METHODS</b>By referring to three diagnostic criteria of BSS [practical diagnostic criterion of BSS (criterion A), diagnostic criterion of BSS in 1986 (criterion B), Consensus of Integrative Medicine on BSS Diagnosis in 2011 (criterion C)], 712 patients from different departments of Xiyuan Hospital were recruited. The reliability of criterion A and its consistency with the other two criteria were assessed using Kappa coefficient. A Bayesian approach was also employed to assess the sensitivity and specificity of criterion A.</p><p><b>RESULTS</b>According to the consistency check, criterion A presented good consistency when used by different researchers (the diagnostic accordance rate was 91. 96%, Kappa =0. 82, P <0.001). Meanwhile, there was an acceptable diagnostic consistency among the three diagnostic criteria. Bayesian estimation suggested that criterion A had higher sensitivity but similar specificity, as compared with criterion B or criterion C. Compared with criterion B [the median of sensitivity and specificity were 0. 762 (95% Cl: 0. 731 -0. 790) and 0. 902 (95% Cl: 0. 858 -0. 936) respectively, the median of sensitivity and specificity of criterion A were 0. 911 (95% CI: 0. 888 - 0. 930) and 0. 875 (95% CI: 0. 826 - 0. 915) respectively. Estimating the difference between criterion A and B, the median of sensitivity and specificity were 0. 149 (95% CI: 0. 112 -0.184) and -0. 026 (95% CI:-0. 085 -0. 033) respectively. Compared with criterion C [the median of sensitivity and specificity were 0. 831 (95% Cl: 0. 804 -0. 857) and 0. 892 (95% CI: 0. 848 - 0. 926) respectively], the median of sensitivity and specificity of criterion A were 0. 912 (95% CI: 0. 889 -0. 932) and 0. 880 (95%CI: 0. 833 - 0.919) respectively. Estimating the difference between criterion A and C, the median of sensitivity and specificity were 0. 081 (95% CI: 0.047 - 0.114) and -0.011 (95%CI: -0.070 -0.046) respectively.</p><p><b>CONCLUSION</b>Compared with criterion B and C, criterion A not only had better reliability, but also could significantly improve the sensitivity without obviously lowering the specificity.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To explore the correlation between the recurrence of cerebral infarction and aspirin resistance (AR)/Chinese medical (CM) constitutions.</p><p><b>METHODS</b>Totally 413 cerebral infarction patients took Aspirin Enteric-coated Tablet (100 mg per day) while receiving routine therapy, 5 days at least in a week. They were followed-up for 12 months. Aspirin sensitivity (AS) was determined using turbidimetry. CM constitutions among patients with different AS were compared. Ratios of AR patients and AS patients of different CM constitutions in cerebral infarction recurrent patients were compared. Platelet membrane glycoproteins (GP) II b HPA-3 gene polymorphism was detected by polymerase chain reaction (PCR) method. Correlation between recurrence of cerebral infarction and AR, bb genotypes, CM constitutions times AS were analyzed by Logistic regression.</p><p><b>RESULTS</b>Totally 11 patients dropped out, 101 (25.12%)with recurrent cerebral infarction and 301 (74.88%) without recurrent cerebral infarction. There were 152 (37.81%) AR patients and 250 (62.19%) AS patients. AR accounted for 26.6% (80/ 301) and AS accounted for 73.4% (221/301) in non-recurrent cerebral infarction patients. AR accounted for 71.3% (72/101) and AS accounted for 28.7% (29/101) in recurrent cerebral infarction patients. There was statistical difference in AR and AS ratios (χ2 = 64.287, P = 0.000). The proportion of yin deficiency constitution (YDC) was the largest [28.3% (43/152)] in AR patients. The proportion of blood stasis constitution (BSC) was the largest [23.6% (59/250)] in AS patients. There was statistical difference in CM constitutions between AR patients and AS patients (χ2 = 21.574, P < 0.01). The former 4 recurrent rates occurred in AR patients of YDC, BSC, damp-phlegm constitution (DPC), qi deficiency constitution (QDC). YDC occupied the first place [22.4% (34/152)]. The former 4 recurrent rates occurred in AS patients of BSC, QDC, DPC, damp-heat constitution (DHC). BSC occupied the first place [3.2% (2/250)]. Compared with non-recurrent cerebral infarction patients and AS patients, bb gene occurred most often, but aa gene and ab gene occurred obviously lesser in non-recurrent cerebral infarction patients and AR patients (χ2 = 20.171, χ2 = 55.139, P < 0.01). AR and bb gene were positively correlated with recurrent cerebral infarction (OR = 18.423, P = 0.000; OR = 1.304, P = 0.028). Body constitutions interacted with AS (OR = 0.707, P = 0.000).</p><p><b>CONCLUSIONS</b>Recurrent cerebral infarction was closely related to AR and constitutional types. The recurrence rate was higher in AR patients of YDC. GP I b HPA-3 bb genotype might be a risk factor for AR and recurrent cerebral infarction.</p>

Objective To investigate the expression and prognostic significance of leucine-rich and immunoglobulin-like domains 1 (LRIG1) and epidermal growth factor receptor (EGFR) in cervical adenocarcinoma.Methods The expression levels of LRIG1 and EGFR were assayed by immunohistochemistry in 47 cervical adenocarcinoma,24 cervical glandular intraepithelial neoplasia (CGIN) and 60 normal cervical tissues.Results The expression rates of LRIG1 in cervical adenocarcinoma,CGIN and normal cervical tissues were 21.28 % (10/47),29.17 % (7/24) and 83.33 % (50/60),respectively,the expression of EGFR in cervical adenocarcinoma,CGIN and normal cervical tissues were 82.98 % (39/47),45.83 % (11/24),5.00 % (3/60),respectively.The expression of LRIG1 was not correlated to age (P > 0.05).However,it was correlated to histological grade,lymphatic nodes metastasis and clinical stages (all P < 0.05).The survival rate of positive LRIG1 patients was higher than that of negative LRIG1 patients (P < 0.05).Conclusions LRIG1 may play an important role in predicting clinical outcomes of cervical adenocarcinoma,and its mechanism may be related to inhibiting EGFR signal transduction.

ABSTRACT

Objective To investigate the expression of stratum corneum chymotuptic enzyme (SCCE) and its significance in human cervical adenocarcinoma.Methods Immunohistochemistry was used to detect the expressions of SCCE,ER,CEA and Vimentin in tissues from the group of 45 cervical adenocarcinoma patients,group of 24 cervical glandular intraepithelial neoplasia (CGIN) patients and group of 24 normal cervical tissues.Results The expression of SCCE was 84.44 % (38/45),58.33 % (14/24) and 8.33 % (2/24) in 3 groups respectively.The SCCE expression from group of cervical adenocarcinoma were more significantly than that of normal cervical tissues (P < 0.012 5),the expression of SCCE from group of CGIN were more significantly than that of normal cervical tissues (P < 0.012 5),but there was no difference between the cervical adenocarcinoma and the CGIN on the expression of SCCE (P > 0.012 5).Conclusion SCCE could be recommended as a new biomarker for the early screening cervical adenocarcinoma and as an additional marker for the prognosis of cervical adenocarcinoma.

ABSTRACT

Objective To detect the expression of Survivin and hypoxia inducible factor-1α (HIF-1α) in prostatic hyperplasia and prostatic carcinoma,and investigate their roles and mutual correlations in pathogenesis and progression of prostatic carcinoma.Methods The expression of Survivin and HIF-1α in proliferative lesions of prostate (15 cases) and prostatic carcinoma (62 cases) were detected by immunohistochemistry method.The relationship between the expressions of Survivin and HIF-1α was analyzed,as well as their correlations with clinicopathological features.Results The positive expression of Survivin and HIF-1α were significantly higher in the prostatic carcinoma [71.0 % (44/62) and 69.4 % (43/62),respectively] compared with those of the prostatic proliferation [6.7 % (1/15) and 0] (x2 =20.56,P 0.001; x2 =23.56,P =0.001,respectively).The expression of Survivin and HIF-1α in prostate carcinoma was significantly related with the histological grading,clinical staging (x2 =10.64,5.39,7.62,6.43,all P < 0.05).And there was positively correlated between Survivin and HIF-lα (rs =0.350,P =0.006).Conclusion Survivin and HIF-1α synergistically play important roles in pathogenesis and progression of prostatic carcinoma.

<p><b>OBJECTIVE</b>To explore effects of exercise combined Danzhi Jiangtang Capsule (DJC) on the protein expression of nicotinamide adenine dinucleotide phosphate (NADPH) oxidase p22phox in pancreatic tissues of diabetic rats.</p><p><b>METHODS</b>Sixty male Wistar rats were injected with low dose of streptozotocin and fed with high fat diet to establish a diabetic rat model. The levels of p22phox and 8-hydroxy-2-de-oxyguanosine (8-OHdG) protein in pancreatic tissues were detected by immunohistochemical method, and the level of p22phox protein was also detected by Western blot in the normal group, the model group, the excise group, the DJC group, and the DJC +excise group, respectively.</p><p><b>RESULTS</b>The expression levels of p22phox and 8-OHdG protein in pancreatic tissues were significantly higher in the model group than in the normal group (P <0.01). p22phox and 8-OHdG were mainly expressed in the cytoplasm of pancreatic cells. After administration of exercise or DJC, the expression lev- els of p22phox or 8-OHdG protein in pancreatic tissues decreased significantly (P <0. 01). Exercise combined DJC had synergistic effects in decreasing expressions of p22phox and 8-OHdG (P <0.05).</p><p><b>CONCLUSION</b>Exercise, DJC, and exercise combined DJC could protect islet beta cells by decreasing the expression of NADPH oxidase in beta cells and reducing sources of oxidative stress.</p>

<p><b>OBJECTIVE</b>To compare the safety and efficacy of RevoLix 120 W 2 µm continuous-wave (cw) laser enucleation of the prostate with transurethral resection of prostate (TURP) in patients with symptomatic benign prostatic hyperplasia (BPH). And to evaluate clinical value of 120 W 2 µm cw laser enucleation.</p><p><b>METHODS</b>All 168 patients with BPH underwent 2 µm cw laser enucleation (n = 88) or TURP (n = 80) between January 2010 and January 2011. The operative time, drop in hemoglobin, drop in serum sodium, indwelling catheterization time and operative complications were recorded. International prostate symptom score (IPSS), quality of life (QOL), urinary peak flow rate (Qmax) and post-voiding residual urine (PVR) were also compared.</p><p><b>RESULTS</b>The mean operative time was slightly longer in the 2 µm laser group ((63.2 ± 21.6) min) than the TURP group ((59.4 ± 18.6) min) (P > 0.05). Transfusions were not necessary in 2 µm laser group. Catheter indwelling time were (1.8 ± 0.6) days vs. (3.5 ± 2.6) days in 2 µm laser group than in TURP group (t = 3.912, P < 0.05). All cases were followed up for 3 - 12 months, the IPSS, QOL, Qmax and PVR were 6.1 ± 2.0, 4.4 ± 1.6, (18.8 ± 4.8) ml/s and (21.6 ± 16.5) ml in the 2 µm laser group, and were 6.3 ± 2.4, 1.9 ± 1.1, (18.4 ± 4.2) ml/s, (23.2 ± 14.6) ml in TURP group respectively. All the markers were improved significantly compared with that of preoperative in both groups (t = 12.453 - 26.213, P < 0.01), but no statistical differences could be found between the two groups. Perioperative complications were less in the 2 µm laser group.</p><p><b>CONCLUSIONS</b>The 120 W 2 µm cw laser enucleation is an novel excellent treatment for BPH as well as TURP, and has the advantage of significantly less blood loss, shorter hospitalization, shorter catheter indwelling time and rapid recovery after surgery.</p>

ABSTRACT

To discuss the effects on early warning of measles, using the neural networks.Based on the available data through monthly and weekly reports on measles from January 1986 to August 2006 in Wuhan city. The modal was developed using the neural networks to predict and analyze the prevalence and incidence of measles. When the dynamic time series modal was established with back propagation(BP) networks consisting of two layers, if p was assigned as 9, the convergence speed was acceptable and the correlation coefficient was equal to 0.85. It was more acceptable for monthly forecasting the specific value, but better for weekly forecasting the classification under probabilistic neural networks (PNN). When data was big enough to serve the purpose, it seemed more feasible for early warning using the two-layer BP networks. However, when data was not enough, then PNN could be used for the purpose of prediction. This method seemed feasible to be used in the system for early warning.

ABSTRACT

<p><b>OBJECTIVE</b>To determine the relationship between the rate of condom use and incidence of Chlamydia amongst commercial sex worker, using a mathematical model.</p><p><b>METHODS</b>Assuming that p(%) is the rate of condom use by female sex workers, and r(%) is the incidence of Chlamydia. If the use of condom increases by delta p, then the incidence of Chlamydia will decrease by delta r. k is the relative rate of change. Then, the mathematical model established becomes dr/dp = -kr.</p><p><b>RESULTS</b>The solution of the differential equation is r (p) = r (P0) exp [-k (p - p0)]. Using the surveillance data gathered from 100% Condom Use Program in Wuhan City, the k value is calculated to be 4.36. If k indicates the contribution coefficient of reducing Chlamydia after condom use, when the rate of condom use increases by 16%, then the incidence of Chlamydia will decrease by 50%. The average difference between the actual incidence and the incidence calculated from the mathematical model is only 6.2%. This result demonstrates a good fit. The predicted result of using this mathematical model shows that at the time of lower levels of condom use, a small increment on the rate of condom use would considerably reduce the infection rate of Chlamydia.</p><p><b>CONCLUSION</b>When k remains constant, this mathematical model reflects the qualitative relationship between the rate of condom use and the incidence of Chlamydia.</p>

ABSTRACT

Objective To evaluate whether the number of high intensity transient signals(HITS)intraoperative detected by transcranial doppler ultrasound(TCD)can predict postoperative neuropsychological deficit.Methods Twenty patients scheduled for coronary artery bypass grafting(CABG)operation with cardiopulmonary bypass were enrolled in this study.HITS were recorded by TCD intraoperatively.And neuropsychological test battery was used to assess postoperative neuropsychological changes.Results There was no significant difference in the number of HITS compared with patients with or without neuropsychological changes.And multivariable correlation between the test scores and the number of HITS showed there was a weak correlation between RW Dgt-Dg5 and HITS(P<0.05).Conclusion The number of HITS detected during cardiopulmonary bypass(CPB) do not!correlate with the postoperative neuropsychologial changes.
